Strabismus therapy

Strabismus (crossed eyes) can be treated at each age. Especially in babies and young children therapy has to be initiated as soon as possible. Visual development happens mainly in the first year of life and matures until 8 years of life. Most of the affected children can be treated sufficiently by using glasses, after measuring their dioptries in cycloplegy. The remaining cases nead occlusion therapy, prism therapy and sometimes an eye muscle surgery.

This early treatment guarantees full visual acuity on both eyes, improves binocular vision and long lasting success.

Furthermore orthoptic specialists support the ophthalmologic team and offer possibilities beyond normal eye doctors practice.

Why do patients squint?

Each eye is moved by six eye muscles. This interaction can be disturbed by many factors like an imbalance in the eye muscle tonus. Thus if the brain cannot learn in the first 12 month of life to bring both pictures of the eyes together (fusion) or fusion is not possible because the visual axes cannot be moved parallel, the picture of one eye is suppressed (in children) or older patients experience double vision.

Eye muscle surgery

Eye muscle surgery can be done after children have accepted their glasses and amblyopia treatment with occlusion therapy has brought some improvement. (i.e. patching of one eye) .

How is an eye muscle surgery done and what can be reached with an eye muscle surgery?

By shortening, elongating and repositioning of the eye muscle tendons just right below the anterior conjunctiva a new balance in the eye muscle forces is established. The tendons can be reached by micro incision of the conjunctiva (minimal invasive eye muscle surgery). Thus the recovery period is very short and the patient can leave the hospital 3-4 hours after the operation. Usually an operation takes 20-45 minutes. I recommend an operation under general anesthesia, in adults the surgery can also be done under local anesthesia. Especially trained children anesthesists take care of our young patients.

ORTHOPTIC TREATMENT, VISUAL TRAINING

The orthoptic treatment and visual training includes a complete examination of about 60 minutes by an orthoptic specialist and an ophthalmologic examination.

Please bring your glasses, previous medical reports and an overview of your medications.

Examinations:

  • visual acuity, measurement of your glasses.
  • biomicroscopic examination with a slit lampe to evaluate tear ducts, eye lid function, tear film, cornea, lense and all anterior parts of the eye.
  • eye pressure with applanation tonometrie, correction with pachymetrie of cornea thickness
  • ophthalmoscopy of the posterior parts (retina, optic nerve, blood vessels, macula). In some cases a medical mydriasis of the pupil is required. (Please in this case DO NOT drive automobiles).
  • evaluation of binoculr vision, motility, strabismus diagnotic, stereo acuity, 3D vision,....

What can be expected from refractive laser treatment?

Refractive laser treatment uses modern techniques to correct effectively refractive errors of the eye. Most people can expect a life without glasses and/or contact lenses.

We offer the combination of the VISX STAR S4 iris recognition excimer laser system with the Femtolaser-technology iFS™n. With this well-established machine all modern operation techniques like LASEK, iLASIK and femto-iLASIK can be provided.

The shape of the cornea is changed by ablating the tissue of the corneal stroma, under the corneal epithelium, which is kept mostly intact to act as a natural bandage.

cataract

A cataract is a progredient clouding of the lens inside the eye which leads to a decrease in vision.
It is most commonly due to biological aging but there are a wide variety of other causes. Senile cataract is caused by deposition of yellow-brown pigment within the lens and this, together with disruption of the normal architecture of the lens fibers, leads to reduced transmission of light, which in turn leads to visual problems. Typical symptoms  are difficulty appreciating colors, changes in contrast, driving, reading, and problems coping with glare from bright lights. Modern cataract surgery has made enormous progress and is therefore a relativ pain free and short operation. Typically it is done in topical anesthesia on an outpatient basis. Through a corneal microincision of 2,2mm the lense is removed (phacoemulsification) and an artificial lense (IOL) is implanted . By adapting the refractive power of the IOL a previous refractive error can be reduced nearly completely.

A previous ambulant consultation is mandatory.

The operation technique is discussed in detail, the different lense models can be explained, and after a complete eye check the power of the lense is calculated with the IOL Master from Zeiss. Thereafter the operation appointment can be made and the final documents for the operation day at the private clinic Confraternität Josefstadt (Skodagasse 32, A – 1080 Vienna) are send to you by post or eMail.

thyroid-associated ophthalmopathy

Thyroid-associated ophthalmopathy is one of the most typical symptoms of Graves' disease. Fibroblasts in the orbital tissues express the Thyroid Stimulating Hormone receptor (TSHR). This may explain why one ans the same autoantibody to the TSHR can cause disease in both the thyroid and the eyes. An autoimmune mediated inflammation affects the orbital contents including the extraocular eye muscles, the orbital fat and the connective tissue.
The ocular manifestations include soft tissue inflammation, proptosis (protrusion of one or both globes of the eyes), corneal exposure, eye lid retraction, motility problems (diplopia) and in rare cases compression of the optic nerve.

The orthoptic treatment and visual rehabilitation training includes a complete eye check by Prof. Dorner and is followed by therapy sessions of about 60 minutes with certified orthoptic colleagues.

Icare - Tonometer Kinder Augendruckmessung
